Workers’ Compensation Underwriting Requirements

• Submitted applications must include a supplemental application, 3-4 years of currently valued loss runs and an updated Acord 130
• No submission will be accepted without all required documents
• Underwriting will respond as promptly as possible

Commercial Auto Underwriting Requirements

• New or Renewal submissions must include Commercial Auto ACORD applications – (ACORD 125, ACORD 127, ACORD 137)

• Supplemental general underwriting questions completed in Arrowhead Exchange
• MVRs on all drivers, including those driving their personal vehicle on company business due at time of submission
